Are you living with diabetes or know someone who is? Take control of your health through a free and evidence-based program developed at Stanford University. The free Diabetes Self-Management Program (DSMP) consists of a highly interactive six-week series. Covered topics include: preventing complications, problem solving, the importance of healthy eating, exercise, medication management, communicating with healthcare providers, dealing with stress and depression, and much more. The free workshops are open to anyone. All attendees must register by the second workshop session. Each participant (one per household) will receive a compliemntary book, Living a Healthy life with Chronic Conditions.
